Tryout time is a busy time of the season administrative wise, so please be patient and refrain from sending multiple emails asking for your PTS or completing this form more than once. The PTS form will not be made available from OMHA about 7 days prior to AAA tryouts. ONLY SUBMIT the FORM ONCE. As Soon as PTS forms are made available you will receive your form by email. DO NOT SEND MULTIPLE EMAILS ASKING FOR YOUR PTS FORM
